' Provisional Application' for My Website '

Dear Sir,

I'm interested in 'My Website' and wish to get my personal website developed and hosted.
I agree to pay the required charges of Rs.10000/- towards the cost of development
and hosting. Further, I agree to pay the yearly maintenance / web hosting charges from
second year onwards.

I'll submit the list of required details, information, etc along with the payment of Rs.10000/-.

My details are as under     :
